To make the check-in process faster, please print and complete the following new patient forms before your first visit. You may fax completed forms to us or bring them with you to your appointment.
Please bring an accurate and updated list of your prescription and over-the-counter medications, your dental insurance information, and any questions you’d like to ask the dentist. We allow ample time for a discussion about your health history and what you hope to accomplish with your dental health today and in the future.
Addisville Dental accepts most PPO dental insurance, and we are happy to file claims on your behalf. If you’d like a treatment estimate before a procedure, please ask! We are pleased to provide a quote ahead of your appointment.
We offer many payment options to meet your needs. If a payment plan is something that interests you, please inquire. We work with patients on a case-by-case basis to create an arrangement that works for your unique situation.
